Step-Test Waiver

The step test involves physical exertion. Stop immediately and tell the operator if you feel chest pain, dizziness, unusual shortness of breath, pain, or feel unwell. This station provides a wellness fitness estimate, not medical diagnosis or emergency care.

I confirm I am fit to participate today and have no known reason to avoid light-to-moderate exertion. I understand and accept the inherent risks of participating in the step test.
